ABSTRACT

Purpose -- In this research work, emphasis is made on collecting society response and acceptance levels of technologies developed and transferred by TVET Institutions. Critical information on the technical and performance parameters of transferred technologies was received from many respondents who were closely associated with society. Design/methodology/approach -- The methodology included questionnaires based on 5-point Likert scale questionnaire with an objective to ascertain the acceptance levels of technologies developed and transferred by TVET Institutions. The questionnaire contained queries addressing variety of issues, like type of technologies developed and developing, manpower participating in technology development, problems associated with technology development and transfer. The methodology followed was based on the analysis of the response received on technical questionnaire that was designed and responses received from the people located in Addis Ababa city Practical Implications: COVID-19 has been rising and mutating in many countries since it was discovered by the end of 2019. Many countries, including Ethiopia has seen the spike and fall of COVID-19 regularly. This non-linear behavior of COVID-19 infection rate curves around the world has prompted many educational institutions like TVET (Technical Vocational and Education Training) institutions located in Addis Ababa to develop and transfer technologies like hand washing machines, face masks, sanitizers and other anti-covid technologies. These equipment's posse's characteristic simple operational and quality properties like usability, functionality, efficiency, etc. This research will help to gather the technical information pertaining to design, quality and performance of Hand washing equipment's, face masks, Hand sanitizers and other equipment's from the society. Findings - Divergent and constructive opinions were received from the society to improve and meet the ever-rising demand for anti-COVID-19 technologies. The user response on developed and transferred technology prove the usefulness to combat COVID-19 spread but also finds many applications with in the life of the public. Cronbach alpha reliability test value of 0.73 was obtained after analysis of the response data. The results prove the excellence and consistency of the responses received on the development and transferred technologies. Overall, the research findings help to build and transfer most robust, quality and cost-effective technologies that is a need of the hour to fight against COVID-19 transmission. Originality/value -- COVID-19 has posed many challenges to public and has resulted in many deaths in Addis Ababa and entire Ethiopia. ABSTRACT

Purpose --The main purpose of this research work is to evaluate the efforts of TVET efforts' in developing and transferring useful technologies in Addis-Ababa, Ethiopia. TVET Institutions are engaged in developing and transferring technologies like hand washing devices, sanitizer sprayers, ventilators, beds, disinfection devices, sanitizers, antiviral finish fabrics, masks and hand-free devices that is used to prevent contamination of the human body while working with materials of different kinds. These equipment's posse's characteristic simple operational and quality properties like usability, functionality, efficiency, etc. The evaluation of these developed and transferred technologies help to prevent transmission of COVID-19 at community level. Design/methodology/approach -- The methodology used in this research was descriptive and purposive sampling type. The sample pool consisted of 5 TVET institutions. 40 respondents participated in this questionnaire study. The response was recorded through interview questionnaires based on the 5-point Likert Scale. Data analysis and Cronbach's alpha reliability tests were computed using SPSS and Minitab software. Practical Implications: The coronavirus is the deadly pandemic and highly contagious disease the human mankind has witnessed since 1918 flue pandemic. However, the coronavirus spread in Ethiopia at the community level has prompted many TVET (Technical Vocational and Education Training) institutions and universities to ramp up their efforts to develop and transfer technologies. Hence TVET organizations need therefore to develop and transfer technologies that are useful to prevent the spread of COVID-19. This research will help to gather the technical information pertaining to design, quality and performance of Hand washing equipment's, face masks, Hand sanitizers and other equipment's. Findings - The results prove TVET Institutions' efforts were successful in developing and transferring the technologies required to combating COVID-19. Cronbach's alpha (reliability test) value is 0.77 for the TVET colleges, indicating the data is excellent, unique and consistent. The position of the TVET institution is excellent as regards to the efforts put in developing and transferring technologies used for combating COVID-19 in and around Addis Ababa. The responses received were unique in nature. Originality/value -- COVID-19 has posed many challenges to public and has resulted in many deaths in Addis Ababa and entire Ethiopia. ABSTRACT

Background: The COVID-19 pandemic has increased mental health problems among healthcare workers globally. However, studies from low- and middle-income countries on this matter were minimal. This study assessed the change in depression prevalence during the first year of the COVID-19 pandemic and associated factors among healthcare providers in Addis Ababa, Ethiopia. Methods: We conducted surveys among healthcare workers in Addis Ababa at two-time points, September 2020 and October 2021. A total of 577 study participants were randomly selected based on registers obtained from professional associations. Computer-assisted telephone interviewing technique was used for data collection. The Patient Health Questionnaire (PHQ-9) was used to screen for depression. Multivariable logistic regression analysis was performed to identify potential factors associated with depression. Result: The prevalence of depression among healthcare workers was 2.3% (95% CI [1.1-4.8]) in Time 1 and 6.5% (95% CI [4.1-10.1]) in Time 2; nearly a three-fold increase in Time 2 compared to Time 1. The most frequently reported symptoms at both times based on the PHQ-9 item were having poor energy, sleep problem, and anhedonia, while reported suicidal ideation was less than 5%. Depression showed a positive and significant association with a positive COVID-19 test result (AOR 7.25 95% CI [1.32-39.4]) in Time 1, and with being a female healthcare provider (AOR 3.96 95% CI [1.08-14.51]) and lack of COVID-19 related policy or guidelines at the workplace (AOR 3.22 95% CI [1.11-9.35]) in Time 2. Conclusion: The prevalence of depression among healthcare workers tripled during the first year of the COVID-19 pandemic. Panic reaction to positive COVID-19 test result seems to have a negative effect at the beginning while lack of disease-specific prevention guidelines and comprehensive psychological interventions for healthcare providers had an adverse effect on the mental health of health workers.

ABSTRACT

Background: Although diffuse alveolar damage and respiratory failure are the most common symptoms of coronavirus disease 2019, other organ involvement, such as the kidney, has been reported. The incidence of acute kidney injury in COVID-19 patients has been reported to vary greatly. In this study, we look at the magnitude and risk factors for acute kidney injury in COVID-19 patients in Ethiopia, a developing country. Methods: A hospital-based retrospective cross-sectional study design was conducted among admitted COVID-19 patients at Eka Kotebe general hospital and Saint Peter COVID-19 treatment center by reviewing data from September 2020 to September 2021. A random sampling technique with proportional size allocation was used to select a total sample of 402 patients (225 from Eka Kotebe and 177 from St. Peter). Secondary data was collected from patient medical records using a standard, pre-tested data collection checklist using the Kobo toolbox, which was then exported to SPSS version 25.0 for analysis. The association between dependent and independent variables was analyzed using binary logistic regression. A statistical significance test was declared at a p value of <=0.05 with a 95% confidence interval. Results: A total of 402 patient charts were reviewed, and the proportion of patients with acute kidney injury was found to be 18.9%. After adjusting for potential confounders, age<=35 years (AOR = 0.23, 95% CI = 0.07-0.72), female gender (AOR = 0.51, 95% CI = 0.28-0.94), and isolation type ICU (AOR = 5.11, 95% CI = 1.44-18.06) were significantly associated with acute kidney injury. Conclusion: Acute kidney injury is a common complication in hospitalized COVID-19 patients. The prevalence of acute kidney injury in this study was 18.9%. Age, gender, and type of isolation were the factors that had a significant association with acute kidney injury. Clinicians and other concerned parties should provide more care to ICU patients and COVID-19 patients who are older.

ABSTRACT

This study examined the acceptability of the COVID-19 vaccines and measured adherence to non-pharmaceutical interventions among employees in public transportations in Addis Ababa, Ethiopia. In a public transportation company-based cross-sectional study, a self-administered questionnaire or a structured face-to-face interview was used to obtain information about willingness to get vaccinated, adherence to recommended non-pharmaceutical interventions, and source and quality of information about COVID-19 vaccines. Overall, 23.8% of 412 responding employees were willing to receive a COVID-19 vaccine. A majority (75.2%) never used facemasks, had poor knowledge about COVID-19 vaccines (82.3%), and had the opinion not to be susceptible to COVID-19 (81.1%). Better education increased the odds of being willing to get vaccinated (OR = 3.28, CI: (1.24-8.63)), male sex (OR = 2.45 (1.08-5.58)), history of chronic disease (OR = 3.01 (1.38-6.56)), watching TV for information on COVID-19 (OR = 14.79 (2.53-86.62)) or considering COVID-19, a severe disease (OR = 9.12 (3.89-21.35)). In addition, the opinion that vaccination can prevent COVID-19, trust in the vaccines, and assuming COVID-19 has an impact at the workplace increased the odds to accept vaccination significantly. In contrast, poor knowledge about the COVID-19 vaccines reduced accepting vaccination significantly (OR = 0.20 (0.09-0.44)). Acceptance of COVID-19 vaccines among public transportations workers in Addis Ababa is very low, which may be due to insufficient knowledge about the vaccines, cultural factors, religious beliefs, and a lack of or distorted information about the disease. Therefore, stakeholders should provide credible and tailored information to transportation workers on the severity and impact of COVID-19 and inform them about the effectiveness of the vaccines.

BACKGROUND: Severally ill COVID-19 patients may require urgent transport to a specialized facility for advanced care. Prehospital transport is inherently risky; the patient's health may deteriorate, and potentially fatal situations may arise. Hence, early detection of clinically worsening patients in a prehospital setting may enable selecting the best receiving facility, arranging for swift transportation, and providing the most accurate and timely therapies. The incidence and predictors of abrupt prehospital clinical deterioration among critically ill patients in Ethiopia are relatively limited. STUDY OBJECTIVES: This study was conducted to determine the incidence of sudden clinical deterioration during prehospital transportation and its predictors. METHODS: A prospective cohort study of 591 COVID-19 patients transported by a public EMS in Addis Ababa. For data entry, Epi data V4.2 and SPSS V 25 were used for analysis. To control the effect of confounders, the candidate variables for multivariable analysis were chosen using a p 0.25 inclusion threshold from the bivariate analysis. A statistically significant association was declared at adjusted relative risk (ARR) ≠ 1 with a 95 % confidence interval (CI) and a p value < 0.05 after adjusting for potential confounders. RESULTS: The incidence of prehospital sudden clinical deterioration in this study was 10.8%. The independent predictors of prehospital sudden clinical deterioration were total prehospital time [ARR 1.03 (95%; CI 1.00-1.06)], queuing delays [ARR 1.03 (95%; CI 1.00-1.06)], initial prehospital respiratory rate [ARR 1.07 (95% CI 1.01-1.13)], and diabetic mellitus [ARR 1.06 (95%; CI 1.01-1.11)]. CONCLUSION: In the current study, one in every ten COVID-19 patients experienced a clinical deterioration while an EMS provider was present. The factors that determined rapid deterioration were total prehospital time, queueing delays, the initial respiratory rate, and diabetes mellitus. Queueing delays should be managed in order to find a way to decrease overall prehospital time. According to this finding, more research on prehospital intervention and indicators of prehospital clinical deterioration in Ethiopia is warranted.

Background: In response to the COVID-19 pandemic, the Ethiopian Ministry of Health has established ambulances and dispatch centers specifically designed to transport COVID-19 patients to city medical centers. Due to a lack of evidence in this area, it is critical to assess the factors that influence the willingness to call for emergency medical services. Methods: A community-based cross-sectional study was conducted from June to July 2021 in Addis Ababa. The data were gathered through use of a pretested questionnaire and analyzed using SPSS 25. Logistic regression was used to calculate odds ratios with and without confounding variable controls, and significant associations were declared at (0.05). For the variables that show significant associations in bivariate and multivariate analyses, a 95% confidence interval is provided. Results: Three-quarters of those surveyed said they would call ambulance if they experienced worsening COVID-19 symptoms. The individual's language was associated with the willingness to call EMS for worsening COVID-19 symptoms [AOR 0.51(95% CI: 0.28-0.92)].; awareness of the availability of toll-free ambulance services [AOR 3.4(95% CI: 1.92-5.95)]; recalling an EMS number [AOR 4.3(95% CI: 1.71-10.67)]; ambulance crew quality of care perception [AOR 3.6(95% CI: 2.09-6.10)]; ambulance service adequacy and accessibility perception [AOR 0.25(95% CI: 0.11-0.55)]. Conclusion: The study showed significant portion of the Addis Ababa community recognized the need to call EMS due to worsening COVID-19 symptoms. The individual's language, awareness of toll-free ambulance; ability to recall an ambulance number, perception of the ambulance crew's quality of care and adequacy and accessibility of service in the city influence the decision to call an ambulance. The finding highlights the significance of addressing problems through various media outlet, for advocacy and public awareness. More research, including qualitative studies, is needed to investigate the factors that encourage people to seek EMS.

ABSTRACT

The global expansion of urbanization is posing associated environmental and socioeconomic challenges. The capital city of Ethiopia, Addis Ababa, is also facing similar threats. The development of urban green infrastructures (UGIs) are the forefront mechanisms in mitigating these global challenges. Nevertheless, UGIs in Addis Ababa are degrading and inaccessible to the city residents. Hence, a 56 km long Addis River Side Green Development Project is under development with a total investment of USD 1.253 billion funded by Chinese government aid. In phase one of this grand project, Friendship Square Park (FSP), was established in 2019 with a total cost of about USD 50 million. This paper was initiated to describe the establishment process of FSP and assess its social, economic, and environmental contributions to the city. The establishment process was described in close collaboration with the FSP contractor, China Communications Construction Company, Ltd. (CCCC). The land use changes of FSP’s development were determined by satellite images, while its environmental benefits were assessed through plant selection, planting design, and seedling survival rate. Open and/or close ended questionnaires were designed to assess the socioeconomic values of the park. The green space of the area has highly changed from 2002 (8.6%) to 2019 (56.1%) when the park was completed. More than 74,288 seedlings in 133 species of seedlings were planted in the park. The average survival rate of these seedlings was 93%. On average about 500 people visit the park per day, and 400,000 USD is generated, just from the entrance fee, per annum. Overall, 100% of the visitors were strongly satisfied with the current status of the park and recommended some additional features to be included in it. In general, the park is contributing to the environmental and socioeconomic values of the city residents, and this kind of park should be developed in other sub-cities of the city as well as regional cities of Ethiopia to increase the aesthetic, environmental and socioeconomic values of the country, at large.

Background: Coronavirus disease 2019 (COVID-19) has resulted in severe acute respiratory failure, requiring intubation and an invasive mechanical ventilation. However, the time for initiation of intubation remains debatable. Therefore, this study aimed to compare early and late intubation on the outcome of COVID-19 patients admitted to the intensive care unit (ICU) of selected Addis Ababa COVID-19 treatment centers, Ethiopia. Methods: A multicenter retrospective cohort study was conducted on 94 early and late intubated ICU-admitted COVID-19 patients from October 1, 2020, to October 31, 2021, in three selected COVID-19 treatment centers in Addis Ababa, Ethiopia. A simple random sampling technique was used to select study participants. An independent t-test, Mann Whitney U test and Fisher's exact test were used for statistical analysis, as appropriate. A P value < 0.05 was used to declare a statistical significance. Results: A total of 94 patients participated, for a response rate of 94.68%. There was a statistically insignificant difference in the rates of death between the early intubated (47.2%) and the late intubated (46.1%) groups (P = 0.678). There was no difference in the median length of stay on a mechanical ventilator (in days) between the groups (P = 0.11). However, the maximum length of stay in the ICU to discharge was significantly shorter in the early intubated (33.1 days) than late intubated groups (63.79 days) (P < 0.001). Conclusion: Outcomes (death or survival) were similar whether early or late intubation was used. Early intubation did appear to improve length of ICU stay in ICU-admitted COVID-19 patients.

A sero-prevalence study was conducted to estimate prior infections with SARS-CoV-2 in Addis Ababa. Methods: A cross-sectional study was conducted from April 23 to 28, 2020 among 301 randomly selected residents of Addis Ababa;sub-city health offices, health facilities and health extension workers were contacted, to obtain a population profile and to conduct the random selection of study participants. Participants were selected, who had not been in direct contact with people who had contracted COVID-19, to maintain consistency among the study population. Interviews on socio demographic and behavioural risk factors, followed by serological tests were performed for SARS-CoV-2 IgM, and IgG antibodies, using the COVID-19 IgG/IgM Rapid Test Cassette. Based on the manufacturer information, the test has a sensitivity of 87·9% and specificity of 100% for lgM;and a sensitivity of 97·2% and specificity of 100% for IgG. A Polymerase chain reaction (RT-PCR) test was also done on combined nasopharyngeal and oropharengeal swabs. Findings: The unadjusted antibody-based crude SARS-CoV-2 prevalence was 7·6% and the adjusted (weighted average) SARS-CoV-2 prevalence was estimated at 8·8% (95% CI 5·5%-11·6%) for the study population. Higher sero-prevalence were observed for males (9.0%), age below 50 years (8.2%), students and unemployed (15.6%), as well as those with primary education (12.1%), educated above high school (37·9%), non- smokers (78·7%), with no history of regular alcohol (53·8%), no chat (70·8%), and no shisha use (94·7%). According to the findings, a significantly higher number of individuals had been infected in Addis Ababa as compared to what was being detected and reported by the RT-PCR test, which is suggestive of community transmission.

ABSTRACT

AIM: This study assessed the level of adherence to antiretroviral drugs and the associated factors among clients who have a follow-up at public health facilities in central Ethiopia. METHOD: A multi-site cross-sectional study was conducted from August 1-30, 2020 at seven public health institutions. A systematic random sampling method was used to recruit 385 participants. Data was collected using a structured interviewer-administered questionnaire. Analysis was done using descriptive statistics, and binary logistic regression model. The OR with its 95% C.I was employed to present analytic outputs. Statistical significance for the multivariable model was considered at p ≤ 0.05. RESULTS: Of the 371 participants, the majority were females (233, 62.8%), attended health centers (215, 58.0%), and were married (173, 46.6%). Eighty-nine (89, 24.0%) of the participants have at least one comorbidity. About 72 (19.0%) and 50 (13.5%) of the respondents stated that the COVID-19 has posed challenges on their follow-ups and availability of medications respectively. Nearly a half of the people living with HIV and comorbid T2DM or hypertension (29, 48.0%) reported that they had encountered an increase in the price of medications compared to the pre-COVID-19 times. About half of the respondents in the study setting have perfect adherence to antiretroviral therapy (ART) (200, 54.0%). Basic education (aOR = 3.02: 95% CI: 1.57-5.80), marriage (aOR = 2.27: 95% CI: 1.24-4.15), attendance to a health center (aOR = 0.59: 95% CI: 0.36-0.98) and sleep disturbance (aOR = 0.47: 95% CI: 0.26-0.84) showed a statistically significant association with adherence to ART. CONCLUSION: About half of the respondents in the study settings have perfect adherence to their ART medications. As multiple factors interplay in the success rate of adherence to ART, stakeholders should place and strengthen practices, such as active follow-up and tracing of cases, ensuring medication affordability (access and low pricing), and psycho-social support to patients.

ABSTRACT

Since the beginning of the year 2020, the world has been suffering from an unprecedented situation due to the Corona Virus Disease (COVID-19). The negative impact of COVID-19 is one of the worrisome issues across the globe. Among others, employment is one area affected during the COVID-19, which requires considerable scientific studies to identify factors affecting employment status throughout the disease crisis. Therefore, this study has mainly aimed to investigate the factors affecting the employment status during the COVID-19 pandemic in Ethiopia, taking a total of 2,396 respondents who had jobs before the COVID-19 outbreak. To achieve the stated objectives, the study has employed a binary logit regression model considering the employment status of respondents who lost their job (unemployed) and who secured their job (employed) during the pandemic. The model result indicates that females were more likely to be unemployed than males, persons living in a rural area were more likely to be unemployed than persons living in an urban area, and persons engaged in industry, service, and trade were more likely to be unemployed than people engaged in agriculture during the pandemic. Furthermore, during the pandemic, people living in the capital city of Ethiopia (Addis Ababa) were more likely to be unemployed compared to people living in the other regions of the country. Finally, based on these findings, critical recommendations were forwarded to the government and policymakers for their intervention.

Findings: The unadjusted antibody-based crude SARS-CoV-2 prevalence was 7.6% and the adjusted (weighted average) SARS-CoV-2 prevalence was estimated at 8.8% (95% CI 5.5%-11.6%) for the study population. Higher sero-prevalence were observed for males (9.0%), age below 50 years (8.2%), students and unemployed (15.6%), as well as those with primary education (12.1%), educated above high school (37.9%), non- smokers (78.7%), with no history of regular alcohol (53.8%), no chat (70.8%), and no shisha use (94.7%). According to the findings, a significantly higher number of individuals had been infected in Addis Ababa as compared to what was being detected and reported by the RT-PCR test, which is suggestive of community transmission.

ABSTRACT

In this paper, we have presented the medical devices maintenance campaign experience conducted in Ethiopia as part of COVID-19 emergency response. We have described the approaches and methods followed before and during this maintenance campaign. We have also discussed the results of the maintenance campaign and its cost implications. The paper briefly describes the challenges faced during the maintenance campaign. Finally, this article reveals few gaps identified in medical devices management, utilization and provides essential recommendations. The purpose of this paper is to share the experiences and measures taken to increase the availability of functional medical devices during COVID-19 pandemic when all medical resources have been limited globally.

ABSTRACT

Background: The aim of this study was to assess the health-related quality of life of patients with T2DM and hypertension attending public health facilities in Addis Ababa, Ethiopia. Methods: A cross-sectional study was conducted from 1st through 30th August 2020 at the selected institutions. Health facilities were chosen purposively based on patient load. Participants were drawn after proportional to size allocation. A translated EQ-5D-3L, and EQ-VAS instrument was used to collect the data. Analysis was done using SPSS v.26.0. Both parametric and non-parametric models were applied in the analysis. Results: Of the 409 participants included, the majority were in the age group of 46-60 (36.0%), females (56.0%), from hospitals (54.8%), jobless (25.4%), and married (63.3%). Over two-thirds of the patients reported no problems with self-care, usual activity, and depression/anxiety. All dimensions showed an increasing proportion of moderate to severe problems in the age group beyond 45. Facility type, comorbid condition and age showed a statistically significant score difference for QoL. The overall prevalence of any problem was 59.0%. Education level, visit to a health center, and marriage showed lower odds of affected HRQoL whereas, lower monthly income and presence of comorbidities were opposite. Conclusion: HRQoL of patients in the study settings was suboptimal and below the general population. Attributes, such as education, facility type, marital status, income level, and comorbid status have a statistically significant association with HRQoL. Arrangement of a safe and quality health services is paramount, especially, during the COVID-19 pandemic.

ABSTRACT

Background: The aim of this study was to determine the magnitude of abnormal organ function tests and biomarkers in hospitalized patients with confirmed COVID-19 and to define the association among markers of organ failure, disease severity and its outcome in hospitalized COVID-19 patients in Ethiopia. Methods: A prospective cohort study was conducted among COVID-19 patients admitted to Millennium COVID-19 Treatment Center from December 2020 to June 2021. Results: The median age of the 440 study participants was 60.3 ± 1.3 years, and from these 71.3% of patients were male. Disease severity: p-value: 0.032; adjusted odds ratio (AOR) (95% CI): 4.4 (0.022-0.085); and the presence of any co-morbidity; p-value: 0.012; AOR (95% CI): 0.80 (0.47-0.83) was significantly associated with mortality. Aspartate transaminase, alanine transaminase and alkaline phosphatase parameter values of patients overall, were elevated - mainly among critical patients (56.9 ± 57.7, 58.5 ± 63 and 114.6 ± 60, respectively).

ABSTRACT

BACKGROUND: In many settings, health care service provision has been modified to managing COVID-19 cases, and this has been affecting the provision of maternal and child health services. The aim of this study was to assess trends in selected maternal and child health services performance in the context of COVID-19 pandemic. METHODS: A cross-sectional data review was conducted in Addis Ababa, Ethiopia from April to May 2021. Routine health management information system database was reviewed from Addis Ababa Health Bureau for the period from July 2019 to March 2021 across all quarters. Proportion and mean with standard deviation were computed. T-test was used to assess statistically significant differences in services mean performance. RESULTS: Postnatal care  visit, new contraceptives accepters, safe abortion care and number of under-5 years old children treated for pneumonia significantly decreased by 9.3% (p-value 0.04), 20.3% (p-value 0.004), 23.7% (p-value 0.01) and 77.2% (p-value < 0.001), respectively during the first 8 months of the COVID-19 pandemic compared to the previous 8 months' average performance. The trends in Antenatal care first visit, new contraceptive accepters, pentavalent-3 vaccination and under-five children treated for pneumonia began to decline in January to  March 2020, a quarter when the COVID-19 pandemic began; with accelerated declines in April to June 2020 following national lockdown. The trends for the stated services began to increase during July-September 2020, the last quarter of national lockdown. Contraceptive accepters and pentavalent-1 vaccination continued to decline and showed no recovery until January-March 2021 when this study was completed. CONCLUSIONS: Most of the maternal and child health services performance declined following the onset of COVID-19 pandemic and national lockdown, and most of the services began recovering during July-September 2020, the last quarter of national lockdown. However, new and repeat contraceptive accepters and pentavalent-1 recipients continue to decline and show no recovery during end of the study period. Implementing COVID-19 prevention measures and assuring the community about the safety of service delivery is imperative to ensure continuity of the maternal and child health services. Regular monitoring and evaluation of services performance is required to identify slowly recovering services and respond to potentially volatile changes during the COVID-19 pandemic.

ABSTRACT

BACKGROUND: Coronavirus disease 2019 (COVID-19) has resulted in unprecedented morbidity, mortality, and health system crisis leading to a significant psychological destress on healthcare workers (HCWs). The study aimed to determine the prevalence of symptoms of common mental disorders among HCWs during the COVID-19 pandemic at St. Paul's Hospital, Ethiopia. METHODS: A self-administered cross-sectional study was conducted to collect socio-demographic information and symptoms of mental disorders using validated measurement tools. Accordingly, PHQ-9, GAD-7, ISI, and IES-R were used to assess the presence of symptoms of depression, anxiety, insomnia, and distress, respectively. Chi-square test, non-parametric, and logistic regression analysis were used to detect risk factors for common mental disorders. RESULTS: A total of 420 healthcare workers participated in the survey. The prevalence of symptoms of depression, anxiety, insomnia, and psychological distress was 20.2%, 21.9%, 12.4%, and 15.5% respectively. Frontline HCWs had higher scores of mental health symptoms than non-frontline healthcare workers. Binary logistic regression analysis showed that being married was associated with a high level of depression. Furthermore, working in a frontline position was an independent risk factor associated with a high-level of symptoms of depression, anxiety, and psychological distress. LIMITATIONS: It is a single-center cross-sectional study and the findings may not be nationally representative or reveal causality. CONCLUSIONS: A significant proportion of healthcare workers are suffering from symptoms of mental disorders. Frontline HCWs were at a greater risk of severe symptoms. Therefore, psychological interventions should be implemented to support health professionals, especially frontline workers.
